All patients are entitled to receiving fair dental treatment. Whether treatment is rendered in a timely manner or not depends solely on the patient. The FamLee Dentist PLLC makes it our goal to deliver dental care professionally and responsibly. The patient is responsible for their own oral health and understands that services rendered do not have lifetime guarantees. All treatment is meant to restore either active or inactive disease processes for the dental longevity of each individual. As such, the continuation of home oral care is critical in maintaining existing or future treatment.

As a condition of treatment by this office, financial arrangements must be made prior to services rendered (i.e. out-of-pocket, insurance-based, etc.). Any emergency dental services or any dental services performed without previous financial arrangements must be paid for at the time of service.

Patients with dental insurance understand that all dental services are charged directly to the patient and that they are personally responsible for payment. This office will help prepare the patients insurance forms and assist in making collections from insurance companies and will credit patients accounts accordingly. However, this dental office cannot render services on the assumption that our charges will be paid by an insurance company.

A service charge of 2% per month (24% per annum) on the unpaid balance will be charged on all accounts exceeding 60 days, unless previously written financial arrangements have been made between the practice and the patient.

The practice reserves the right that any fee estimate for dental services rendered can only be extended for a period of six months from the date of the patient's examination.

The FamLee Dentist PLLC charges $25 for returned checks. We also reserve the right to charge a $50 fee for missed appointments not cancelled 24 hours in advance.

The patient is financially responsible for all charges whether or not paid by insurance. Should you default on payments of your account and collections agency services are required, all cost of collection including attorney fees will be added to the balance of your account.
